What Is TMJ Dysfunction?

TMJ stands for temporomandibular joint — the hinge joint that connects your jaw to your skull, just in front of each ear. You have one on each side, and they work together every time you talk, chew, yawn, or swallow. When something goes wrong with the joint itself, the surrounding muscles, or the way they coordinate together, the result is what's broadly called TMJ dysfunction or temporomandibular disorder (TMD).

It's worth knowing that "TMJ pain" isn't one single problem — it's a category of related issues that can have different causes and different presentations. That's a big part of why a thorough evaluation matters so much before jumping into treatment.

Symptoms That Might Bring You to Us

TMJ dysfunction can show up in a surprising number of ways. Some people come in with obvious jaw pain, but others aren't sure their jaw is involved at all until we dig into it. Common symptoms include:

  • Pain or tenderness in the jaw, face, or in front of the ears

  • Clicking, popping, or grinding sounds when opening or closing the mouth

  • A jaw that locks open or closed

  • Difficulty or discomfort when chewing, yawning, or speaking

  • Headaches — especially in the temples or at the base of the skull

  • Ear pain or a sensation of fullness in the ears

  • Neck pain and stiffness

  • Facial muscle tightness or fatigue

How We Treat TMJ Pain

TMJ dysfunction responds really well to physical therapy when it's done right — and "done right" means treating the whole picture, not just the joint in isolation. The jaw doesn't operate independently from the rest of your head and neck, and our treatment reflects that.

Manual Therapy Hands-on treatment is at the core of what we do for TMJ patients. This can include gentle joint mobilizations to restore normal jaw movement, soft tissue work on the muscles of the jaw and face, and treatment of the neck and upper spine — which are more connected to your jaw symptoms than most people realize. Some manual therapy techniques are performed inside the mouth (with gloves, of course) to directly address the muscles responsible for jaw movement.

Therapeutic Exercise We'll guide you through specific exercises to restore normal jaw movement, improve the coordination of the muscles that open and close your mouth, and address any postural factors that may be contributing to your symptoms. These are exercises you can do at home and that make a real difference when done consistently.

Dry Needling For stubborn jaw and neck tension, dry needling can be a game-changer. It sounds intimidating, we know — but most of our TMJ patients tell us it was nothing like they expected, and the relief they feel afterward makes it well worth it.

Education Understanding your own jaw goes a long way. We'll talk about habits that may be aggravating your symptoms (clenching, gum chewing, nail biting, resting your chin in your hand), help you understand what's driving your pain, and give you the tools to manage flare-ups on your own.

A Note on the Team Approach

For some patients, physical therapy works best as part of a broader care team. We're comfortable collaborating with your dentist, oral surgeon, or other providers when that's what your situation calls for. If you're already working with someone for your TMJ — whether that's a dentist fitting you for a nightguard or a specialist managing other aspects of your care — we're happy to be part of that team and communicate with them directly.
